6

Close the menu (Escape or navigate away) — confirm focus returns to a logical place

7

10

Tab through Checklist Library filter buttons — confirm all are reachable without skipping

11

Tab through footer links — confirm they appear after main content, not before

12

Confirm focus indicator is visible on every focused element throughout the flow

"Stop and Fix" Signs

If any of these occur during testing, note the page and element for review.

Focus disappears entirely — you can no longer see where focus is

Tab gets stuck — pressing Tab does not move forward

Mobile menu cannot be closed using only the keyboard

Skip link does not appear on first Tab press

Skip link does not jump to main content when activated

A form field has no visible label before or near the input

A button shows only an icon with no visible text or accessible label

Emergency or safety content is hidden behind a required interaction

Copy or Print buttons cannot be reached without a mouse

Main content area has no id — skip link has nowhere to land

ⓘ

Focus order should also move through form labels, grouped option labels, and validation messages in a logical order. See Form Label Review, Fieldset and Legend Review, and Error Message Review.
